Job Overview
We are seeking a dedicated and compassionate Physical Therapist to join our team. The ideal candidate will have experience in outpatient clinics, and will be responsible for providing exceptional patient care to individuals across different age groups. The Physical Therapist will develop and implement effective care plans tailored to each patient’s needs, utilizing a range of therapeutic techniques.
We believe that a physical therapist should provide the care for our patients, so all treatment sessions are one-on-one with our patients. We see patients both in and out of network with insurance. Treatment sessions are 45 minutes. We do have access to modalities, but tend to spend most of our clinic time with manual therapy or exercises. We think it’s important that the patients understand why we are doing particular treatments, so education is a big part of the plan of care as well.
Duties

Conduct thorough assessments of patients’ physical abilities and limitations.

Develop individualized treatment plans based on patient evaluations and goals.

Implement therapeutic interventions, with one-on-one treatment sessions.

Monitor patient progress and modify treatment plans as necessary.

Provide education to patients and their families regarding treatment options and home exercises.

Collaborate with other healthcare professionals to ensure comprehensive patient care.

Participate in discharge planning to facilitate smooth transitions for patients leaving the clinic and returning to PLOF or sports.

Maintain accurate documentation of patient progress and treatment outcomes.
