A new year brings fresh starts, new goals and a reset for many insurance plans. If your health insurance runs on a calendar-year cycle, January marks the point when deductibles, out-of-pocket maximums, and visit limits reset. That’s why it’s especially important to provide your updated insurance information at the beginning of the year.

1. Deductibles Reset Every January

For most calendar-year plans, your deductible returns to $0 met on January 1st. This means you will once again be responsible for the full cost of covered services until your deductible is met. If you notice your cost-share increase at the start of the year, this is likely why. It’s part of the normal annual cycle. Once the deductible is met, you will likely have a copay, which is a set dollar amount per service, or a coinsurance, which is a percentage of services that you are responsible for.

2. Out-of-Pocket Maximums Restart Too

Just like your deductible, your out-of-pocket maximum also resets in January. Until you reach this maximum, you may experience higher costs for visits or treatments. Once the maximum is met, your plan typically covers 100% of approved services for the rest of the year.

3. Visit Limits Renew

Many insurance plans include visit limit maximums for certain therapies or treatments. These limits also renew at the start of the year. If you previously reached your max number of visits in 2025, you are likely to have access to a fresh set of eligible visits for 2026.
